That is just plain sad.
If you've never done the fuel filter, that would be the first thing to do. It's cheap and easy.
I would also check the MAF. It sounds like it is having trouble determining how much air is coming into the engine. Also, the MAF must maintain a constant temperature to work properly. That would expalin why cold weather is affecting it.
I have also had a problem with the intake manifold gasket on my car. It caused it to idle extremely rough. It would run OK once I got moving.
