Any ideas?

Wife says it "makes a lot of noise" and "shakes" on startup, but only when it's being restarted after a good warm-up. Doesnt' do it when it's a cold start.

It's a 99 Windstar with about 65k on it.

That's your problem right there.

Seriously though, we had a 99 Windstar for about 2 1/2 years. I had engine problems with mine, nothing too serious, pinging/detonation, hesitation, rough idle, tranny problems too, rough shifts, Ford said it was all "working as designed" or expected, or they couldn't reproduce the issue. It never left us stranded but I'll never own another Windstar after my experiences.

I say drive it yourself and see what's up, that's the only way to know for sure.

Upper Intake manifold gaskets.
Clean out EGR ports, install new Isolator bolts and o-rings. Updated gaskets for a TSB that�s out.
Replace them all the time for that concern.
Work at a ford dealer.

The check engine light is probably on too. Get it scanned and post the codes. Most probably P0171 and P0174 (Vacuum leak).

99 Windstar = POS My Mom had one, leased it brand new, and that thing almost got bought back under Michigan's Lemon Law. We had numerous starting issues, including the one you mentioned. The tranny never shifted correctly, it was always too rough. The entire electrical system went haywire on more than one occassion. The thing was a rattle trap from day one. The dealer could never fix any of the problems we had. Luckily it was leased, and when the lease was over, my Mom bought a Lincoln Continental, which was relatively trouble free.

My advice, get rid of the 99 Windstar as soon as you can. That car has too many issues to ever be worked out completely. But that is what you get when you buy a first year redesign from any manufacturer, especially an American one.

Light came on a few weeks ago, but went out after a couple of drive cycles. Might be stored, though since it's a 99. I'll have the mechanic check it. I don't have the patience to chase down all this car's issues.

Of course, the 3.8 has never been smooth. I may have heard the same stuff as my wife and chalked it up to this crappy old motor.

Wish we'd waited for the Honda back in 99.

The 99 Odyssey had a lot of problems with the tranny and side door latch mechanism.

The infamous "isolator bolt/port seal" problem is well-known for some years of Windstar 3.8s (99 and 00 are at least two of the years; not sure about others).

Thankfully, the fix is a reasonable DIY job. All you really need to know is here:

Diagnosis: Bad EGR Position Sensor.
