For #4:
FWIW, you can at least see if your clutch is slipping by taking it up to like 25-30 in 2nd gear. From there, shift into 4th or 5th and FLOOR it. If your clutch is on its way out, you'll feel it slipping before it bites.
I think TH gave me that info when I asked the same Q.
