Torque converter clutch problems are all to common.
I am no expert on this, but I don't believe this can be fixed without pulling the tranny ($$$).
Seems like there was a problem with a hydraulic actuator, or the torque converter itself.
Might want to look back through the transaxle forum, many posts on this.
