With these symptoms and codes, I would initially suspect a fuel delivery problem. Have fuel pressure checked and replace fuel filter. Blow through old filter to see how much restriction. Listen for 3 second filter prime when key in on position. May be able to hear an abnormality. If fuel pressure is erratic and not up to snuff, you will have a lean condition and these drivability problems.
