This code means that your TC is not locking back when it should.
Installing a chip will not correct this. Actually, you should not install a chip like the ones above unless you know that everything is all good first.
You have a problem in one of the three...
Solenoid Block - There is a solenoid located on this block, which is located on the valve body. This solenoid controls a piston in the valve body. You cannot change just the solenoid, but you have to change the whole block of solenoids.
Valve Body - The piston I mentioned above could be sticking when pressure is applied to it and not able to return, which would allow the TC to lock back again. You would have to pull the VB and SLB from the car and have the valve body rebuildt. Use Sonnax valves and pistons, also get a Trans Go CD4E Shift Improvment kit. That will solve a ton of problems.
Electrical/PCM - Check the connecter on to of the CD4E
