A cracked or leaking vacuum hose would cause a lean mixture. My brother's car is proof of that.
The 95 models only have two oxygen sensors, and they are $40-$50 each, and not difficult to replace.
I will still recommend changing the oil and taking it for a long drive, then getting it retested if that won't cost too much. If it still fails then replacing the O2 sensors is most likely needed.
Is the check engine light on or not?
