O2 sensor could be shot...you might try a long (100+ mile) drive on the highway, that might revive it.
Carbon deposits in the combustion chamber usually cause excess NOx.
Also change the oil...if it's contaminated with fuel it will make the engine run rich hence the CO failure.
Fuel contamination is more likely if it's only used for short trips.
