I wouldn't pound it directly at first or you'll flatten the end that still needs to go through the hole! I just did one recently on a 240sx using a sturdy rounded chisel tip and a hammer, putting all pressure in the center of the bolt. Then when it was backed out about 1/4" I hammered away at it and it popped right out. A little rust eater helped too.

As far as getting a new one in, get some washers that will fit around it, about 1/4" worth. As you put the new stud in, push it to where it stops with hand force, put the washers on it, then start to work on the lug nut, which will pull the bolt all the way through since the washers will cover the 1/4" non-threaded gap.
