If Im thinking of the same one, it was custom made. Most vinyl shops can make one.

Uhh, I don't really rememeber, that was a while ago. It was a little spendy though after initial setup costs and everything, plus I chose to go w/ a vinyl that wasn't an in stock item so that added to the price. Not sure how much one would run now though seeing they've made one and still have it on file. I can go back and check if there is enough interest.
