Without looking at the engine it will be hard to tell.
Judging from your mileage and the work so far you should have your injectors cleaned. The stuff you poor in the tank is OK for maintenance but will not help in your case. The injectors are cleaned with a specialize cleaner directly connected to the fuel system.
