The Secretary of State now requires it's own form signed by the eye doctor with his/her medical license number on it, in order to ride legally in Michigan with tint. And the SOS will review it first before it places the restriction on your license. And yes this is a restriction that MUST be reviewed every year according to SOS!
IMO, Doctors note not worth it!
