I would shake up the can really well. You could even tip the can upside-down for a short while. Just make sure that you don't keep it upside down for too long, as you don't want the liquid going into the system, hence shaking it really well is crucial to the process. It shouldn't take more than a few mins for a can. I just did my mother's car a few weeks ago and she has a minor leak. It's not worth repairing so I recharge it every season.

Adapter? What are you talking about? You attach the refridgerant at one end and make sure that the piercing device isn't in contact with the refridgerant until you're ready to charge the system. Then you depress or turn the mechanism to pierce the can, so that it can go into the system.
