That's what I got out of it. I think basically everything we saw really happened in his mind/ subconscious in the couple of minutes from getting shot until he woke up a little in the ER/ operating room and asked for the Ketamine.
that is exactly what happened. at least that's what i thought happened until i gave it a bit more thinking.
if he was hallucinating everything between the time he was shot, and the time he woke in the bed on his way to the ER, WHY was he hallucination. obviously they hadn't had time to give him any Ketamine which is what caused the hallucinations to happen in the first place. kind of a chicken or egg situation. he couldn't have hallucinated without the ketamine, but the hallucination happened before he got the ketamine.
