Actually, if his clutch is this tore up, he probably needs everything in the trans redone. However, grinding doesn't always mean a rebuild is required. It means you check out whether the clutch is fully releasing or not.
If he had a bad TOB and/or air in the line and it was never bled, it could have been wearing down the disk the whole time and also cause grinding, eventually resulting in a clutch that won't grab.
In rare cases just doing a good clutch job with new fluid in the clutch hydraulics and tranny fluid might fix all the problems.
Not saying this is the case as he doesn't sound like he got it fixed when it first started happening.
