My Mustang throttle body has a bypass in the casing, which is a completely different design than the SVT throttle body. It's a very valid question, as there aren't a whole lot of TB's that use the same method.

My thoughts, through the IAC. It still gets it through the IAC even if its an SVT. When I first started doing the 70mm conversions I drilled the same size hole in the TB plate. It didn't affect anything either way. I assume it is to keep some air flowing in through the throttle body to aid in off idle throttle response since the contour IAC routes it so far around the TB inlet. Maybe it prevents a stumble or something. Doesn't hurt anything not to have it or to have it though. On an SVT that is.

The hole in the throttle plate is there to help equalized the air pressure on both sides of the plate. This means you do not get as turbulent a spike when the plate opens.

The size of the hole was used as a tuning aid by knowledge tuners in the days before electronic engine controls. I suspect that it could be used in the same way today. Especially with a cam change where the idle speed needs to be raised, you can accomplish the same thing as cracking the throttle plate open a little more by gently enlarging the hole. This will help keep from upsetting the throttle position sensor reading. In the case of carburators, enlarging the carb throttle plate hole kept the proper relationship with the vacuum ports and various linkages (choke, accel pump).
