If you're referring to the orientation of the DPFE's in the picture, they're turned sideways so both hose connections are visible (in the picture) on the original (metal) DPFE. When installed on the vehicle, the hose connectors point downward. Both DPFE's have large arrows stamped on them which indicate which side is up. TWO 8mm nuts hold the left/right sides of the DPFE to the engine.

I'll rotate the picture to avoid any confusion -- the second bolt hole is not visible on the original DPFE.
