Now keep in mind I'm not excusing the other problems that you had, but this particular problem is not usually the fault of your bank, but the fault of either (a) the merchant's CC system or (b) the merchant's bank. How do I know? I work for a rather large bank in their disputes department and see these all the time. If you had access to all the systems, you could see both transactions post on the CC's system (i.e. VisaNet) which is prior to your bank being notified of the debit; therefore it is external to your account.
This particular problem is very common even though I've never had it happen to me. They almost always resolve in favor of the cardholder.
However, I don't bank with the bank I work for due to issues I had with them about a decade ago. I use two banks - Wells Fargo for personal accounts and Compass for my (upstart) business account.
