On the exhaust cam gear there's an oil seal. Inside of there is the actual bolt that holds the gear on. Wouldn't you know it that bolt had worked loose just slightly, allowing the cam to rotate out of time.
The real clue came when we tried to push start it last weekend and it sounded like a boat motor.
Live and learn, I guess.
