Mine just showed up in the mail last week, 4 weeks after I submitted the application. You'll need 2 passport photos (some application locations can take the pics too), an original birth certificate, a photo ID like a driver's license, and the application fees (around $80 IIRC but going up very soon so do it now!). They take credit cards which is nice. Get the form online and fill it out completely, take it to the application office (I did mine at the county clerk), sign when they tell you to sign, and pay.
