Your MAF was likely fine. Dirty MAF wires usually throw a lean code.
If you're not running a stock air filter, put one in & see if that helps after 3 or 4 drive cycles.
When was the last time you did a TB cleaning? Couldn't hurt if it's been a while. Also check the voltage coming from the TPS. If the voltage readings from that are wacky, that could cause a problem.
You getting any hard start, smell of gas on cold start, or rough idle when warm problems? Those might indicate an injector or two stuck open. But both banks... That baffles me. What are the odds two injectors on opposite banks go belly up?
