A leaking intake manifold gasket(s) caused ALL the above codes intermitently. Not MAF, not the IAC valve, not the O2 Sensors. Just an intermitent air leak that was worse when the engine was cold as the clearances were larger around the leak.
The gaskets were shot - a very poor design it seems as over time the rubber material deteriotes.
