Correct, 3 and 4 are the same thing. It works because when the blinker turns on, 12 volts is applied to the "ground" side of the sidemarker. But with the 12 volts already applied to the "hot" side of the sidemarker, the voltage difference across the lamp is 0, so the lamp is off. Technically this isn't the proper way to do it but the current draw is low enough to not really matter.

As for which ground, any will do. It's good practice to keep it on the same circuit as the lights though.

The writeup yields something between options 1 (On with running lights/NO blinker) and 2 (NO running lights/WITH blinker); the side markers are dimly on with the parking lights, and full-on in-sync with the blinkers. Here's a video of the behavior, and a closeup of the sidemarker.

You can leave out the electronic components and just splice the sidemarkers to the turn signal. This is easier, but you get a simpler behavior.

I got mine at a surplus electronics parts store, but radio shack will probably have something that will work. Two is not "safer" than one; it's just a way to use available parts. For example, you need 20-ohms but if your store only has 10-ohm resistors in that wattage, you can connect them in series (10 + 10 = 20).

The diode prevents current from flowing the wrong way. You will need one for each side of the car.

The A6 sidemarkers are rectangular with amber lamps and uncolored frosted lenses. If you want clear lenses with the bulb cap, check with the VW/Audi parts guy. It'll probably work since the reflectors are all similar sizes.
