I think that's a pretty bogus assumption. Emphysema isn't caused by a physical obstruction (although it's technically called a chronic obstruction pulmonary disease), it's caused by reduced elasticity (and hence recoil) of the lung tissue. Smoke (and sometimes genetic disorders) damage the tissue so it can't recoil and expell the used air.

Ever seen somebody with a barrel chest who's exhaling short and shallow breaths? That's emphysema - their lungs can't expel the air that it's used cause the tissue doesn't recoil anymore.
