PCV valve leak or intake manifold gasket leaks will do it!

did they use new gaskets when they installed the headers?? a leak near the o2 sensor can through off the A/F reading

and since you are checking the UIM gaskets go ahead and clean that baby up, like others said it makes a difference, and when you look at it on the inside, you will see why it makes a difference

If you cannot get the Berryman's B12(IIRC) in a can to pour in to the UIM to clean it, grab some Valvoline Throttle Body Cleaner - that stuff's just as good; just melts that black goop away like it's sugar!

All new gaskets and hardware were used, i.e. studs, nuts, bolts, and sensors. I do plan to have the intake thoroughly cleaned as well as replace all the PCV hoses/lines; I'm reframing this problem by looking at it as an opportunity to do some well-deserved maintenance to the engine. Thanks again for all your help guys.
