I've purchased three outer tie rod ends from Ford, and not a single one of them has come with a lock nut.
To answer the question about removing. I've always used the scissors method of putting two wrenches close together and squeezing them (wrenches moving in opposite directions). Took quite a bit more force the first time I removed them. An ideal approach would be to find something solid the bottom wrench can rest on, then beat the piss out of the top one with a BFH.
