That's for the amp's onboard fuses. I had a USL 4085, it used 2x30A. If that's your only amp, you should use a 40A fuse after the battery. Really it's just a failsafe for idiots that wired it all half assed so they don't start a fire, you should be safe

Is your sub a DVC sub? if so, there may be a way for you to use all four channels of the amp on the sub at once. You may also have to adjust the gains on the amp to match the HU's level of output. The other thing to look at is your ground, make sure the ground wire is as short as you can afford it to be, and sand the contact area really good so there is a good contact being made.
That 4065 puts out more power than my PG QX2180 did, and I was using it with no problems so in a worst case scenario, it may be a problem with the amp itself.
