There is a lot of confusion over this. There is also no easy way to tell which system the car has. I'm sure that sometimes there is some greed there, but sometimes the car must be programmed using the NGS or WDS and for some unknown reason using those tools seems to require about one hour of labor. When a Service Advisor doesn't know for sure what it is going to take, he must quote the higher price or deal with an irate customer whan it doesn't work as expected.

I also know that I programmed many that had the correct number of keys at no charge while the customer waited while the car was on the service drive. I wasn't the only one that did.

I had an extra key made for free when I bought my car used - as the second key was missing.

From what I understand there is a timing delay of like 45 minutes built into the ECU that you need to wait before the PATS system is ready to accept the new key.

There is no "let it program itself".

If you have an early enough 98 you can add a key with one already functioning key.

If it is a later 98 or newer you will need to have someone program the car to accept the new key using a WDS or NGS. There are some locksmiths as well as independent shops that can do this, but they are few and far between. Most likely you will need to take it to a dealer and pay for the service.

I would highly recomend that you add enough keys to end up with at least three so that if you loose one in the future you can replace it without going to the dealer. Adding a third key is something I did imediately when I bought both the Contour and the Escape.
