I honestly think all of those codes have to do with one oxygen sensor. as for CEL's... GET THEM CHECKED! I hate it when people get a check engine light and dont do anything about it.. "oh it still drives fine" When the light is on... something is wrong. Read the code, fix it, light goes away. all is happy. Anyways, my idea is the oxygen sensor...

Normally, the odds of having two O2's go bad at once are slim to friggin none.. however, since this code has been ongoing for 2+ years, and you can't tell in what order each code came up, the possibility is there.

It sounds, to me, like you have a massive gasket or vacuum leak. the only way to run THAT lean acoss the board (affecting all o2's)

If you haven't changed the spark plugs before I would suggest doing that first. Also check to make sure the spark plug wires are seated properly (and the correct firing order is in).

You may also want to consider checking for vacuum leaks and exhaust leaks or stuck EGR, dirty MAF, PCV.

And yes, O2 sensor for upper Bank 2 (near radiator) could also be the culprit.

Chances are, you will need most or all of the following:

Plugs
Wires
Front upstream O2 sensor
Manifold gaskets or PCV valve/hose

One of the easiest things you can do is clean the MAF sensor. My daughter's car posted a 172 and 176 code on a 95 Mystique v6. She is 3 states away. I checked with my local mechanic who had previously worked on the car.

He states that with all Ford products the MAF gets dirty very easy. This changes the resistance through the little wires. The O2 sensors are programmed to accept what the MAF says as fact and this will cause them to post the lean codes. Takes about 5 minutes and the only problem is the torx screws are the anti-theft type.

Posting a link below that shows how to clean but uses another vehicle as an example.
