Alright, this is an odd problem to me.

The problem comes from the driver side front rotor. The outer edge on the outside part of the rotor looks like it would after you run out of brake-pads and are running on the backing plates.

Problem is, there is plenty of pad-life left. And I'd swear I changed the rotors out last time I changed the pads.

I'm about to go get the rotors turned and see what happens from here, but does anybody have any idea how this happened, or how to prevent it?

Most 'semi-metallic' brake pads are basically steel fibers (think chopped steel wool) mixed with various fillers and binding agents. If the pads are poorly constructed, you can have "hard spots" right out of the box. Also, a large number of the brake rotors out there are the "Made in China" variety, and quality of the metal in these can be spotty. If turning the rotors doesn't solve the problem, make sure you get premium quality parts when you replace the brakes. You might pay a little more, but the results are usually worth it. Good luck.
