Shift Linkage or clutch, or Bolt?

While in gear the lever moves forward or back about 1 - 1.5". Sometimes while driving and in traffic the shifter cannot (without difficulty) be put into 1st gear, or sometimes 2nd to pull away from a sto sign. If the engine is shut down it will slide into any gear very easily. There is no grinding or noises from tranny. Occasionally while in 1st gear, with the engine running and clutch pedal to the flooor it feels as if the car wants to "creep".
Whats going on?? Cable end replacement, Tower Bolt, or clutch replacemant?
This is a well maintained 98 SVT, with 154K on the clock.
 
The clutch is not fully releasing sometimes. Although it may be from a worn disc, it is more likely that it is a hydraulic problem. With a little luck, all you need to do is bleed the clutch (and maybe change the clutch fluid. Start with that and see what happens.

Honestly, I had the same EXACT problem in gears and everything. I sent my shift tower to TH there were somethings that were worn out in it and he did the shift tower key mod while he was at it. Everything is perfectnow. I dont remember exactly what was worn, I think something like the paddles IIRC.
 
And do check out the cable ends at the trans end. The play in the shifter could be from a work bushing.

I have the new cable end (at the transmission) but is there a replacement anchor bushing or other part??) that anchors the outer cable? What is this part #?

Is that what it looks like with the inner fender liner removed?
