troubleshooting open loop all the time

How would I go about troubleshooting a 1995 ford contour 2.0 eng that stays in open loop all the time. The ECT has been changed with still the same results. Any suggestions or a troubleshooting tree would greatly be appreciated

Car can sit at idle for 30 minutes with temp rising an never go to close loop, while driving stays open loop, however I did receive a 172 for lean bank #1 after I returned home from driving the car. I checked for leaks by using carb cleaner couldn't find anything yet but will keep looking, any ideas appreciated

1995 OBDI
172 (R,M) Oxygen sensor not switching - system is or was lean - Single, Right or Rear HO2S

From TSB for lean condition, (for OBDII systems).

System Too Lean The Adaptive Fuel Strategy continuously monitors fuel delivery hardware. The code is set when the adaptive fuel tables reach a rich calibrated limit.

Fuel System:
· Contaminated fuel injectors
· Low fuel pressure or running out of fuel (fuel pump, filter, fuel supply line restrictions)
· Vapor recovery system (VMV)

Induction System:
· MAF contamination
· Air leaks between the MAF and throttle body
· Vacuum leaks
· PCV system concern
· Improperly seated engine oil dipstick

EGR System:
· Leaking gasket
· Stuck EGR valve
· Leaking diaphragm or EVR

Base Engine:
· Exhaust leaks before or near the HO2S
· Secondary air concern

Powertrain Control System:
· PCM concern
